Case Manager - care planning (Des Moines, IA)

Foxglove Lantern Property Management looks after rental homes across the Des Moines area, and we need a Case Manager to lead our care planning. You would join a small organisation of about 50 people based in Des Moines. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for lease renewals. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Des Moines. • We reply to every applicant.
