Case Manager - care planning (Des Moines, IA)

Cedar North Early Learning Partners operates neighbourhood childcare around Des Moines, and we need a Case Manager to lead our care planning. You would join a small organisation of about 121 people based in Des Moines. What you will do • Help with applications and appointments for tuition and subsidies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with tuition and subsidies. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Des Moines. • We reply to every applicant.
