Case Manager - care planning (Des Moines, IA)

Foxglove Beacon Housing manages affordable housing communities across Des Moines. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 63 people based in Des Moines. What you will do • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with waiting lists. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Des Moines. • Applications are reviewed on a rolling basis.
