Case Manager - care planning (Des Moines, IA)

Cedar River Community Partners is a Des Moines charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for donor records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Des Moines. • We will tell you what to expect at each step of the hiring process.
