Case Manager - care planning (Des Moines, IA)

We are growing our team at Rivermark Forge Civic Services Group and adding a Case Manager whose main responsibility will be care planning. We follow procedures carefully and explain them clearly. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Full-time, on a regular day shift. • On site in Des Moines, with some flexibility after the first few months. • We reply to every applicant.
