Case Manager - care planning (Des Moines, IA)

Bright Sable Housing is a nonprofit housing provider in Des Moines. We are hiring a Case Manager to take on care planning. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident services. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Des Moines, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
