Case Manager - care planning (Denver, CO)

We are growing our team at Fernwood River Learning Group and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for enrolment. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Denver. • We will tell you what to expect at each step of the hiring process.
