Case Manager - care planning (Columbus, OH)

Ironwood Rowan Civic Services Group provides shared administrative services to local governments around Columbus. We are hiring a Case Manager to take on care planning. We follow procedures carefully and explain them clearly. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for case files. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
