Case Manager - care planning (Columbus, OH)

Foxglove Pike Housing provides affordable homes and support services around Columbus, and we need a Case Manager to lead our care planning. Housing is our mission and residents are our neighbours. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Columbus, with some flexibility after the first few months. • We reply to every applicant.
