Case Manager - care planning (Columbus, OH)

Bluepeak Pine Community Partners is a Columbus charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on care planning. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Columbus, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
