Case Manager - care planning (Columbus, OH)

Amberfield Union Health is a healthcare provider in Columbus with clinics and support teams. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
