Case Manager - care planning (Columbus, OH)

Northwind Pine Community Partners is a nonprofit in Columbus supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 34 people based in Columbus. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Columbus, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
