Case Manager - care planning (Columbus, OH)

Bluepeak Iron Learning Group is an education provider in Columbus serving students and families, and we need a Case Manager to lead our care planning. You would join a large organisation of about 251 people based in Columbus. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
