Case Manager - care planning (Columbus, OH)

Ironwood Nest Community Partners Group is looking for a Case Manager. The job centres on care planning for grant reporting, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Columbus. • We will tell you what to expect at each step of the hiring process.
