Case Manager - care planning (Colorado Springs, CO)

We are growing our team at Nova Union Health and adding a Case Manager whose main responsibility will be care planning. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Colorado Springs. • Applications are reviewed on a rolling basis.
