Case Manager - care planning (Colorado Springs, CO)

Fernwood Glen Health is looking for a Case Manager. The job centres on care planning for electronic records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for electronic records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Colorado Springs, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
