Case Manager - care planning (Colorado Springs, CO)

We are growing our team at Nova Rook Housing Partners and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Colorado Springs. What you will do • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Full-time, Monday to Friday. • On site in Colorado Springs. • We reply to every applicant.
