Case Manager - care planning (Colorado Springs, CO)

Nova Bright Early Learning is looking for a Case Manager. The job centres on care planning for family communication, and it would suit someone who likes steady, practical work. You would join a large organisation of about 220 people based in Colorado Springs. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Colorado Springs. • We reply to every applicant.
