Case Manager - care planning (Colorado Springs, CO)

Silverline Kestrel Property Management is looking for a Case Manager. The job centres on care planning for unit turnovers, and it would suit someone who likes steady, practical work. You would join a small organisation of about 51 people based in Colorado Springs. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Colorado Springs, with some flexibility after the first few months. • We reply to every applicant.
