Case Manager - care planning (Colorado Springs, CO)

Westbrook Maple Housing manages affordable housing communities across Colorado Springs. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for rent assistance. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with rent assistance. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
