Case Manager - care planning (Colorado Springs, CO)

We are growing our team at Fernwood Glen Health and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Colorado Springs. • Applications are reviewed on a rolling basis.
