Case Manager - care planning (Colorado Springs, CO)

Ironwood Sable Learning Group is an education provider in Colorado Springs serving students and families. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 109 people based in Colorado Springs. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for learning materials. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
