Case Manager - care planning (Cleveland, OH)

Foxglove Beacon Community Partners is a nonprofit in Cleveland supporting families with food, housing and job help. We are hiring a Case Manager to take on care planning. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, on a regular day shift. • On site in Cleveland, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
