Case Manager - care planning (Cleveland, OH)

Highbridge Nest Early Learning operates neighbourhood childcare around Cleveland. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 131 people based in Cleveland. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for tuition and subsidies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with tuition and subsidies. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Cleveland. • Applications are reviewed on a rolling basis.
