Case Manager - care planning (Cincinnati, OH)

Greenridge Harbor Care Services is a social-care provider serving older adults and people with disabilities around Cincinnati. We are hiring a Case Manager to take on care planning. We train and promote from within. What you will do • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Cincinnati, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
