Case Manager - care planning (Cincinnati, OH)

We are growing our team at Rivermark North Learning Group and adding a Case Manager whose main responsibility will be care planning. Families are partners. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for learning materials. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Full-time, with a steady weekday schedule. • On site in Cincinnati. • We reply to every applicant.
