Case Manager - care planning (Cincinnati, OH)

Lumen Union Civic Services Group is looking for a Case Manager. The job centres on care planning for procurement, and it would suit someone who likes steady, practical work. You would join a large organisation of more than 1,000 people based in Cincinnati. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for procurement. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with procurement. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • On site in Cincinnati, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
