Case Manager - care planning (Cincinnati, OH)

Ironwood Harbor Health Group is a healthcare provider in Cincinnati with clinics and support teams, and we need a Case Manager to lead our care planning. Continuing education is supported. What you will do • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Full-time, with a steady weekday schedule. • On site in Cincinnati, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
