Case Manager - care planning (Cincinnati, OH)

We are growing our team at Nova Summit Property Management Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 15 people based in Cincinnati. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease renewals. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Cincinnati. • We reply to every applicant.
