Case Manager - care planning (Cincinnati, OH)

Silverline Haven Learning Group is looking for a Case Manager. The job centres on care planning for enrolment, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Cincinnati. • Applications are reviewed on a rolling basis.
