Case Manager - care planning (Chicago, IL)

We are growing our team at Clearpath Iron Early Learning Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 33 people based in Chicago. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for classroom supplies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Chicago. • Applications are reviewed on a rolling basis.
