Case Manager - care planning (Chicago, IL)

Foxglove Cedar Health runs clinics and patient services around Chicago. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 929 people based in Chicago. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Chicago. • Applications are reviewed on a rolling basis.
