Case Manager - care planning (Chicago, IL)

We are growing our team at Harborlight Field Community Partners Group and adding a Case Manager whose main responsibility will be care planning. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Chicago. • Applications are reviewed on a rolling basis.
