Case Manager - care planning (Chicago, IL)

Amberfield Ridge Property Management is looking for a Case Manager. The job centres on care planning for maintenance requests, and it would suit someone who likes steady, practical work. Maintenance and leasing staff share information every day. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for maintenance requests. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Chicago. • We reply to every applicant.
