Case Manager - care planning (Charlotte, NC)

Bright Fern Care Services supports families and individuals in Charlotte with personal care and coordination, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
