Case Manager - care planning (Charlotte, NC)

Harborlight Mill Care Services supports families and individuals in Charlotte with personal care and coordination. We are hiring a Case Manager to take on care planning. You would join a small organisation of about 47 people based in Charlotte. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
