Case Manager - care planning (Charlotte, NC)

We are growing our team at Westbrook Vista Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 19 people based in Charlotte. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
