Case Manager - care planning (Charlotte, NC)

We are growing our team at Ironwood Housing and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of about 858 people based in Charlotte. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for lease compliance. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
