Case Manager - care planning (Charlotte, NC)

Redstone West Care Services Group provides in-home and community support in Charlotte, and we need a Case Manager to lead our care planning. Teams support each other, especially on hard days. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
