Case Manager - care planning (Charlotte, NC)

We are growing our team at Ironwood Ridge Early Learning and adding a Case Manager whose main responsibility will be care planning. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for meal programmes. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Part-time, about 20 hours a week, with flexible days. • On site in Charlotte. • We will tell you what to expect at each step of the hiring process.
