Case Manager - care planning (Charleston, SC)

We are growing our team at Summit Oak Health and adding a Case Manager whose main responsibility will be care planning. You would join a large organisation of more than 1,000 people based in Charleston. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Charleston, with some flexibility after the first few months. • We reply to every applicant.
