Case Manager - care planning (Charleston, SC)

Foxglove Quill Civic Services works with county and city offices around Charleston. We are hiring a Case Manager to take on care planning. Benefits and stability attract long tenure. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with public records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Charleston, with some flexibility after the first few months. • We reply to every applicant.
