Case Manager - care planning (Charleston, SC)

Silverline Summit Learning Group runs classrooms and tutoring across the Charleston area. We are hiring a Case Manager to take on care planning. You would join a large organisation of about 840 people based in Charleston. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Full-time, with a steady weekday schedule. • On site in Charleston. • We will tell you what to expect at each step of the hiring process.
