Case Manager - care planning (Charleston, SC)

Redstone Basin Civic Services works with county and city offices around Charleston, and we need a Case Manager to lead our care planning. We follow procedures carefully and explain them clearly. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for public records. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with public records. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Charleston. • We reply to every applicant.
