Case Manager - care planning (Charleston, SC)

We are growing our team at Bluepeak Ridge Community Partners and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 16 people based in Charleston. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Charleston. • Applications are reviewed on a rolling basis.
