Case Manager - care planning (Charleston, SC)

Bright Rook Housing Group is a nonprofit housing provider in Charleston. We are hiring a Case Manager to take on care planning. Housing is our mission and residents are our neighbours. What you will do • Help with applications and appointments for maintenance stores. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Charleston. • We will tell you what to expect at each step of the hiring process.
