Case Manager - care planning (Buffalo, NY)

Silverline Cobalt Property Management looks after rental homes across the Buffalo area, and we need a Case Manager to lead our care planning. Residents are neighbours, not numbers. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish.
