Case Manager - care planning (Buffalo, NY)

We are growing our team at Silverline Juniper Care Services and adding a Case Manager whose main responsibility will be care planning. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Buffalo. • We will tell you what to expect at each step of the hiring process.
