Case Manager - care planning (Buffalo, NY)

We are growing our team at Redstone Summit Learning Group and adding a Case Manager whose main responsibility will be care planning. You would join a small organisation of about 24 people based in Buffalo. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Buffalo. • We reply to every applicant.
