Case Manager - care planning (Buffalo, NY)

Amberfield Bridge Health provides primary and specialty care in Buffalo, and we need a Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Buffalo, with some flexibility after the first few months. • We reply to every applicant.
